The Attachments tab in the Case allows users to access all documents for the selected case. These files may be Visit or Case forms from the Therapist, or they may be scanned-in documents, such as driver's license and insurance cards from the front office. Any file that is on the computer can be attached to a case.

At the top of the Attachments grid is a Class dropdown. This refers to the Class of the Attachment such as Treatment Evaluation or Non-Treatment. This allows you to view one type of attachment at a time, for example, if you only wanted to see the Treatment Daily Notes.
There is also a checkbox for Related Cases. Checking this box will display all attachments from all cases in a Binder Attachment Group. The items with a white background are on the current selected case and the ones with the grey background are from other cases in the Binder Attachment Group.
The Attachment grid works like a standard Grid report. You can click the Field Chooser in the top left corner to show/hide additional columns.
Attachment Columns
Source | Indicates if the attachment is a Form, Document, or Scanned Image. |
Form Title | If a Form contains this field, the Form Title from the form will appear here. This helps tell the difference between an Evaluation and a Re-Evaluation, etc. |
Description | The user-created description for the item. |
Visit DOS | If the attachment is a visit note (is. therapist documentation) then the visit that the attachment is linked to is displayed. |
Status | Indicates whether the note is signed or not, or if it is unsignable. |
Created | Date and time the attachment was created. |
User | The user that created the attachment. |
CaseID | The Case ID where this attachment resides. |
Service Type | The Service Type of the Case where this attachment resides. |
Form ID | The system-assigned ID of this form allows you to track the form to the form template in the system. |
Extension | Shows the file extension for the file. |
Documents
Any file on the computer may be attached to the case and will appear under the Documents category.
Please note that if you retrieve a document and make changes to it, it will need to be reattached to the case in order for the changes to be saved.
Forms
Forms are part of the Therapist Documentation system and consist of Visit Forms and Case Forms. You can add Case forms here, but Visit forms may only be added from the Schedule or Charges & Documentation.
Scanned
It is possible to scan images directly into the case. These images will appear under the scanned source.
- Right-click in the Attachments tab to access the menu.
Attachment Right-Click Menu
Open | Opens the currently selected form. |
Open Unsigned Clone... | Allows you to create an unsigned copy of the form to which changes may be made. You may then sign this new form. This is useful for making corrections to signed forms that are not changeable. Note, that the new form will have a new creation date. You will now have a record of the original document as well as the newly updated document. |
Move Form | Allows you to move the form to a different visit. This is useful when a note was started for a visit but that visit was subsequently deleted. |
Add Form | New Form | Allow you to add a new Visit or Case form to the case. |
Add Form | New Form Flow Forward | Allow you to flow forward from the currently selected form into a new form of a different or the same type. |
Add Form | New Form Flow Foward to Same | Allows you to flow forward from the currently selected form into a form of the same type. |
Add Document | Allows you to select a document from your computer to save with the case. |
Scan Image | Allows you to scan an image into the case attachments. |
Retrieve to File | Save the stored document to a file. |
Rename Attachment | Pops open to change the Description name of a Document. |
Open Exercise Program | If you are integrated with a Home Exercise Program (exp. PhysioTec), you can launch via this selection. |
Prints the currently selected document(s). | |
Save Form(s) as PDF File(s)... | Saves the Forms (not Documents) as PDF files on your machine. |
Delete | Removes the document from the case. |
Undelete... | Provides a popup of all attachments deleted from the Case. You can select from the dropdown and choose to bring a deleted attachment back. |
