Other Case Info: Diagnoses
The diagnoses area contains a list of all diagnoses related to this case. There can be an unlimited number of diagnoses associated with any particular case, but typically there will be one or two.
Choosing Edit Diagnosis from the right-click menu displays the following dialog:

Deletes the currently selected diagnosis from the case. This will not affect previously charged dates of service or signed Visit forms. NOTE: Deleting a diagnosis before a Visit form is signed will cause the diagnosis to disappear from the list on the form. |
Selecting Add will bring up the following dialog:
The grid will contain a list of all ICD-9 codes currently in your system. Entering a number into the ICD Code text box will filter the list to only those entries that start with the numbers you entered. The description box is a bit more flexible in that it will do an out-of-order word search on all of the descriptions.
If you were looking for ‘Fractured Humerus, upper end’ you could enter ‘Humerus Fracture Upper’ in the text box, and the results would be as follows:
Even though we specified ‘Humerus Fracture’ instead of ‘Fracture Humerus,’ the dialog will still find the related codes.
If the diagnosis you are looking for is not currently in the system, you can click the Add Code button and add one on the spot.
Once you’ve chosen the appropriate code, you can choose the Source and Level. Clicking Save will add this code to the diagnosis list.

Specify the source of the diagnosis as either being a Doctor diagnosis or a Therapist Diagnosis. |

Specify a diagnosis as being a primary diagnosis or a secondary diagnosis. |
NOTE: Each charge code must be associated with at least one diagnosis with a Source of Doctor and a Level of Principal.
